Shake Shack, 5400 West Broad Street – The following violations were reported during a routine inspection: the drive-thru area dish machine ran three times and never reached the required temperature for sanitizing; temperature of chicken nuggets on warming shelf is too low; staff handling food are wearing watches and bracelets; one light is out in the hood system; walk-in cooler fan guards and ceiling near ventilation are dusty; there is trash in floor drain under soda dispenser.

Little Caesars, 9514 Woodman Road – The following violations were reported during a routine inspection: didn’t observe times on prepped pizzas for the oven; observed buildup on racks at buttering station, in walk-in, and in dish area; there are stains and other buildup on walls and floors throughout the facility; purse found on prep table and coats are hanging on a storage rack.

Tropical Smoothie Café, 9972 Brook Road – The following violations were reported during a routine inspection: observed food debris and stickers on containers on clean dish rack; temperatures of chicken and cheese in prep unit are elevated; there is food debris in bottoms of units, including walk-in freezer.

Burger King, 4240 Pouncey Tract Road – The following violations were reported during a routine inspection: employee health policy is unavailable; facility couldn’t provide written procedures for cleaning up vomit and diarrhea; two fryer baskets have broken off metal pieces; there is a y-valve with spray nozzle attached on the mop sink without the proper backflow prevention device installed; observed light dusting surrounding the vent above drive-thru window and above the cook line.

Ciao Capri, 10478 Ridgefield Parkway – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; observed pink buildup inside ice machine at bar; single and prep unit cutting boards are scratched and stained brown; observed dust and grease buildup on sides of fryers, cables behind cook line, and shelves storing clean plates; there is dust buildup on ceiling panels and vents.

Westbury Deli, 8903 Three Chopt Road – The following violations were reported during a routine inspection: facility couldn’t provide written employee health policy or written procedures for cleaning up vomit and diarrhea; observed staff eating and drinking in food prep area; uncovered employee beverage found; there is dried food debris on meat slicer blade and base; temperatures of mayo, spinach and steak in black homestyle refrigerator are elevated; temperatures of tomato, ham and corned beef hash in reach-in cooler are elevated; hunks of previously sliced ham and pastrami are unlabeled; bulk container of sugar/salt is unlabeled; observed fruit/drain flies in dish area (corner under dish machine) as well as one roach near the Coke cooler; food staff aren’t wearing a hair restraint; dishes over three-compartment sink are stacked while still wet; observed dust and mold on reach-in freezer vents; dumpster lids were left open; there are cracked floor tiles throughout kitchen; ceiling tile in kitchen is stained; observed black grime on floors underneath/behind reach-in coolers, dish machine and hot line equipment; wires from ceiling and ceiling vent are dusty.

Thai Flavor Food & Noodle, 4024-D Cox Road – The following violations were reported during a routine inspection: hand sink in front of kitchen is storing empty chemical bottles and is being blocked by a dirty dish cart; temperature of red curry sauce in steam well is too low because of improper reheating methods; large, covered container storing rice in walk-in cooler is not date marked; wire racks in walk-in cooler are peeling; fryer baskets have broken off metal pieces.

Qdoba of Short Pump, 11282 West Broad Street – The following violations were reported during a routine inspection: wire racks in walk-in cooler are starting to peel; drying racks next to three-compartment sink and the dry ingredient storage wire racks are covered in heavy rust; there is a y-valve connected to the mop sink faucet without the appropriate backflow prevention device installed; dumpster area is dirty and surrounded by loose trash and debris.

Gino’s Ristorante Pizzeria, 10118 Brook Road – The following violations were reported during a routine inspection: observed pasta in a large, tightly covered bin three hours after preparation; sliding door refrigeration unit has an elevated temperature; there are torn gaskets on prep units on cook line; observed buildup on walk-in racks and along equipment on cook line; there is food debris on top of dish machine.

Tony’s Italian Restaurant, 3409 Cox Road – The following violations were reported during a follow-up inspection: raw chicken is stored over BBQ pork and salad in walk-in cooler; walk-in cooler gasket is torn.

Grapevine Restaurant, 11055 Three Chopt Road – The following violations were reported during a routine inspection: a worker touched mouth with hand and continued handling food without washing hands; observed brown buildup inside small ice machine; menu doesn’t contain consumer advisory for burgers that can be cooked to order; thawed salmon wasn’t removed from vacuum packaging first; cook is wearing bracelets; wet ice buckets are not inverted in order to air dry; interior lining of large ice machine is torn and falling off; there is heavy ice buildup in white reach-in freezer; observed dust buildup on fan in outdoor storage room and near dish machine; there is food debris in lowboy gaskets near prep table; floor of walk-in freezer is bumpy and raised and there’s a hole in the door.

Panera Bread, 9080 Staples Mill Road – The following violations were reported during a routine inspection: employee talked on their phone outside and then entered kitchen and didn’t wash hands; chicken is thawing above ready-to-eat soup in walk-in cooler; temperatures of sliced tomato and cooked chicken in cold holding prep units are elevated; prepped chicken is cooling while covered in walk-in cooler; several cold holding units are not in use in the facility; observed food debris and grime along perimeter of kitchen.

Taco Bell, 8991 Staples Mill Road – The following violations were reported during a routine inspection: temperature of top layer of refried beans in steam table is too low; front hand sink is coming loose from wall; observed leak at back hand sink faucet.

Salaam Market LLC, 7103 Staples Mill Road – No violations reported during a follow-up inspection.

Jersey Mike’s Subs, 1420 North Parham Road – The following violations were reported during a routine inspection: hand sink is blocked and there is a sanitizer bucket stored in it; quat sanitizer concentration is too low at three-compartment sink; buckets with salt and cornmeal are unlabeled; lowboy cooler has a torn gasket with duct tape on it as well; there is a drip from vent in walk-in cooler (there are bins set up to catch residue); floor fan is dusty; there is a leak from faucet and U-bend at back hand sink; there is a leak from degreaser at mop sink; a y-connector valve on mop sink doesn’t have the proper backflow prevention device installed; floors are dirty under equipment.

Qpot Hot Pot & BBQ, 11740 West Broad Street – The following violations were reported during a routine inspection: bowl of prepared raw shrimp is stored over ready-to-eat sprouts in walk-in cooler; boba found sitting out on counter at room temperature.
