Potbelly Sandwich Shop, 10921-E West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; gasket liners are torn on first lowboy behind checkout counter, first prep unit door on the line, third prep unit door on the line, and walk-in cooler door; long cutting boards on prep units are scored and stained; wire racks in walk-in cooler are covered in old food debris; y-valve and spray nozzle connected to mop sink faucet hose doesn’t have the appropriate backflow prevention device installed; there is dust surrounding all ceiling vents in back kitchen area.
Tropical Smoothie Café, 11736 West Broad Street – The following violations were reported during a routine inspection: observed dark blotches along interior shield of ice machine; plastic bottle containing a yellowish sanitizer chemical is unlabeled; container of light brown powder is unlabeled; right-side door of the prep table beside cash register doesn’t close and stay sealed; wire racks storing dry ingredients are covered in old food residue and powder; dumpster area is dirty and surrounded by loose trash and debris; mop isn’t hung or inverted to air dry.
First Watch, 5310 West Broad Street – The following violations were reported during a routine inspection: scoop for eggs is stored in hot water but the water isn’t hot enough; cutting boards are scored and stained.

Lazeez Modern Indian Flair, 2601-C Tuckernuck Drive – The following violations were reported during a routine inspection: temperature of heavy cream sitting out on cutting board is elevated; bottle of Advil is stored on dry storage shelf; dumpster side door was left open.
Great Steak / Surf City Squeeze, 11800 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; hand sink in front counter area is out of service with no temporary resolution; observed pink slime and mold buildup along interior backside of ice machine; spray bottle containing a clear disinfectant chemical is unlabeled; gasket liner is torn on left-side door of prep unit storing frozen fries and cooked potatoes; two wire racks in walk-in cooler are covered in large patches of rust; ceiling vent above three-compartment sink is surrounded by dust; mop isn’t hung or inverted to air dry.
Funny Bone Comedy Club, 11800 West Broad Street – The following violations were reported during a routine inspection: three-compartment sink is dispensing 0 ppm of quat sanitizer; bulk container of a white powdery substance on dry ingredients shelf is unlabeled; gasket liners are torn on middle door of first prep unit on the line, last prep unit on the line, and first reach-in freezer next to dry storage area; three fryer baskets have broken off metal pieces; facility couldn’t provide chloride sanitizer test strips for the dishwasher.

Flying Dragon, 5314 Chamberlayne Road – The following violations were reported during a routine inspection: imitation scallops is being sold as scallops; staff in kitchen are not wearing hair restraints; multiple foods are stored in non-food grade plastic grocery bags; cutting boards throughout kitchen are scored; cardboard is being reused in kitchen for food storage; door to chest freezer in kitchen is rusted over and the door gasket is torn; wire storage racks in walk-in cooler are rusty and moldy; there is grease residue on sides of hot line cooking equipment; walk-in cooler fan guards and door gaskets are moldy; storage rack liners are dirty; observed food debris on black homestyle refrigerator door gaskets; observed cracked and damaged floor tiles throughout kitchen.
Lokmans Best Wings, 5308 Chamberlayne Road – The following violations were reported during a follow-up inspection: cutting boards in kitchen are heavily scored and stained; observed damaged and stained ceiling tile in dining room.
Quickway Japanese Hibachi, 4501 South Laburnum Avenue – The following violations were reported during a follow-up inspection: observed leak at sprayer faucet on three-compartment sink; prep sink is directly plumbed; hose with sprayer under pressure attached at mop sink doesn’t have the appropriate backflow prevention device installed.

Fresk Wok Restaurant, 5146 Nine Mile Road – The following violations were reported during a routine inspection: uncovered staff beverages found on prep table; person in charge couldn’t present parasite destruction letter from supplier; raw chicken and beef stored above dumplings in reach-in cooler on hot line; raw shell eggs stored above produce in walk-in cooler; food is stored directly on floors in walk-in cooler, walk-in freezer and dry storage; walk-in cooler wire racks and can opener base are rusty; one light is out in the hood system; fryer baskets are in disrepair; cutting boards are stained and scored; observed grease residue and food debris on sides of hot line cooking equipment; walk-in cooler fan guards are dusty; food debris found on hot line and dry storage area prep table tops and bottoms; hot line reach-in cooler door gaskets are dirty; observed leaks at three-compartment sink faucets; there are dried food debris and stains on walls throughout kitchen; ceiling tile in dining area is stained.

Mekong Restaurant, 6004 West Broad Street – The following violations were reported during a routine inspection: there isn’t a certified food protection manager on staff; bar area hand sink doesn’t have paper towels; no handwash signage is posted at beverage station hand sink; food thermometer in kitchen isn’t working; food is stored directly on walk-in cooler floor; tongs and rice paddle are being stored in ambient temperature water in between use; wire storage racks in walk-in cooler are rusty; walk-in cooler fan guards are dirty; reach-in cooler door gaskets throughout kitchen are dirty; observed dust and food debris on wire storage racks; outdoor dumpster doors and lids were left open; there are stained ceiling tiles in the dining area.

AHA Indian Kitchen, 3409 Old Parham Road – The following violations were reported during a follow-up inspection: single and prep cutting boards are heavily scratched and stained; cement floors throughout kitchen and dry storage room are no longer smooth and easily cleanable; observed food debris and grime buildup on floors and walls throughout the kitchen.
