APRIL 22

Hibachi House, 3456 Lauderdale Drive – The following violations were reported during a routine inspection: large container of cooked rice in the three-door reach-in unit began the cooling process two hours ago but still has an elevated temperature; temperature of cooked diced chicken is too low; temperatures of precooked dumplings and raw steak chunks in refrigeration drawer unit underneath flat top grill are elevated; shelf above prep table storing take-out containers and ingredient shelf in the side food prep area are covered in a thick layer of dark debris; gasket liners are torn on right-side and middle door of the three-door reach-in unit; wire racks in the walk-in cooler are peeling; a y-valve and sprayer nozzle are both being used on mop sink faucet without a proper backflow prevention device installed; light shield towards the back of kitchen walkway is cracked.

Papa John’s Pizza, 3067 Lauderdale Drive – The following violations were reported during a follow-up inspection: gasket liners are still torn on middle-right side door of the prep unit and on the door of the standing reach-in unit; there is still a direct plumbing connection underneath the three-compartment sink.

Plentiful Foods, 1145 Gaskins Road – The following violation was reported during a routine inspection: deviled ham spread is past its expiration date.

APRIL 23

Sportsman’s Restaurant & Lounge, 3306 Williamsburg Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; sanibucket found in bar hand sink; there are no dates marked on prepped items (green beans, mashed potatoes and crab); medication found stored on bar cooler and microwave; facility couldn’t provide test strips for sanitizer; observed buildup in soda gun holder and on gaskets and bottoms of units; there are holes in the wall and chipping paint; there are missing coving tiles in bathroom; lights are out in the kitchen; observed mold on wall in kitchen.

Subway, 4386 South Laburnum Avenue – The following violations were reported during a routine inspection: did not observe a certified food protection manager; temperatures of steak and tuna salad prepped over four hours ago are elevated; temperatures of lettuce, spinach, deli meat, chicken and steak on line and deli meat in lowboy are elevated; food prep employees are wearing braclets; racks throughout are rusty.

Papa John’s Pizza at Richmond Raceway, 602 East Laburnum Avenue – The following violations were reported during a routine inspection: dried food debris found on can opener blade; there are no working food thermometers on site; observed dust on ceiling tile and light fixtures near ventilation.

Buz and Ned’s Real Barbecue, 8205 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; hand sink near front corner sink doesn’t reach the required temperature for hot water; quat sanitizer concentration in spray bottles is too high; to-go containers are stored on a dirty surface right-side up; metal is bending and breaking on fryer baskets; single cutting boards are heavily scratched; observed dust buildup on wire storage racks and fan grates in walk-in coolers and on monitor cables; there are broken/missing tile coving in back of kitchen; cement floor at the bar isn’t sealed.

Jadean’s Smokin Six O, 1126 Westbriar Drive – The following violations were reported during a routine inspection: hand sink in outdoor grill area doesn’t have paper towels; observed food debris and grease on prep shelves, lamps, inside keg cooler and inside reach-in cooler; there is dust and grease buildup on ceiling vents in kitchen.

Morrissey’s Catering, 8901-A Three Chopt Road – The following violation was reported during a routine inspection: food residue found on slicer blade.

Hungry Eyes Jamaican Restaurant & Bar, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit or diarrhea; hand sink in the kitchen is temporarily out of service (the only other hand sink in the facility is in the bar area); hand sink in the bar area doesn’t have paper towels; no quat sanitizer test strips are available.

Akoya Kitchen, 10921-F West Broad Street – The following violation was reported during a routine inspection: drying racks across from the three-compartment sink are peeling.

Pizza Hut, 5100 Williamsburg Road – The following violations were reported during a follow-up inspection: wire racks storing takeout pizza boxes are still dirty; all wire racks throughout the facility are still dirty; there is still a direct plumbing connection underneath the three-compartment sink; walls and floors throughout the kitchen are still dirty.

APRIL 25

Ben Wong’s Tea, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: employee drink found on prep table; employee entered from outside and continued work without washing their hands; temperatures of cooked sweet potato filling and cream stored under prep table are elevated; observed a gap between floor and screen door.

America’s Best Wings, 3075-C Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed staff eating food in food prep area; raw chicken is stored over cooked pasta in reach-in cooler; dried food debris found inside microwave, on meat slicer blade and base, and on tomato slicer; observed mold and mildew on exterior of soda machine ice chute; bulk containers of sugar, flour, etc. are unlabeled; food is stored directly on walk-in freezer floor; staff are not wearing hair restraints in kitchen; cutting boards are heavily stained and scored; cardboard is being used to line shelves in kitchen; raw chicken boxes are being reused for storage; facility couldn’t provide sanitizer test strips; kitchen hand sink is covered in dried food debris and mildew; observed dried food debris on reach-in cooler door gaskets and walls behind hot line equipment; wire storage racks throughout kitchen are dirty; walk-in cooler fan guards are moldy; sides of hot line cooking equipment are greasy; kitchen hand sink isn’t draining properly; mop sink with attached hose doesn’t have the proper backflow prevention device installed; ventilation hood filters, fire suppression system pipes and floor underneath deep fryers are greasy; wall behind three-compartment sink has mildew on it; ceiling tile stained in kitchen.

Tortilleria Mixteca, 5172 Nine Mile Road – The following violations were reported during a follow-up inspection: dried food debris found on deli meat slicer blade and base; cloths used for wiping down food contact surfaces are not stored in a sanitizer solution; cutting boards are scored; wood storage rack in walk-in cooler is unfinished; staff drinks found in reach-in cooler amongst food.

APRIL 28

Lehja Restaurant, 11800 West Broad Street – The following violations were reported during a routine inspection: employee health policy is unavailable; first hand sink in the kitchen is being used to fill water containers and rinse dirty cloth linens; observed many black specks and pinkish buildup along interior shield of ice machine; three-compartment sink is dispensing 0 ppm concentration of sanitizer; no date mark found on container of cooked duck in walk-in cooler; dinner menu doesn’t contain a written disclaimer for lamp chops that can be cooked to order; foil is lining the storage shelf and prep table to the left of the cook line; gasket liners are torn on both doors of second prep unit on the line; wire racks in walk-in cooler are peeling and rusty; metal pieces have broken off on one fryer basket; drying rack next to three-compartment sink is covered in old food debris buildup; there is a direct plumbing connection underneath the three-compartment sink; dumpster area is dirty and surrounded by loose trash and debris; mop isn’t hung or inverted to air dry; there is a missing light shield above the cook line.
