Longhorn Steakhouse, 11311-A West Broad Street – The following violations were reported during a routine inspection: box of bread is stored directly on walk-in freezer floor; bread cutting knife is stored in crevice of first prep unit on the line; top shelf of prep line storing take-out containers is covered in a layer of dusty debris; two fryer baskets have broken metal pieces; there is a direct plumbing connection underneath the three-compartment sink; spray nozzle connected to mop sink faucet doesn’t have the appropriate backflow prevention device installed; dumpster area is dirty and surrounded by loose trash and debris; ceiling vents in back of kitchen are surrounded by a layer of dust.
Naz’s Halal Food, 11426 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; no food thermometer is available.
Quarter Horse, 9018 West Broad Street – The following violations were reported during a routine inspection: container of eggnog expired in December; cleaner is stored next to wine; jugs and bottles of oil are unlabeled; walk-in cooler racks are rusty.
Wendy’s, 2200 East Laburnum Avenue – The following violations were reported during a routine inspection: employee is washing a dish in a hand sink meant only for handwashing; dried food debris found inside microwave; water is pooling inside reach-in cooler near drive-thru window; walk-in cooler fan guards are dusty; observed grease residue on sides of deep fryer and fry warmer; walk-in cooler wire racks are dirty; shelf under burger grill is greasy and dirty; there is missing ceiling tile in dining room; ceiling appears to still be leaking water into light fixtures from heavy rain (water droplets pooling in light fixtures); ceiling tile above service counter is dusty; ceiling tile in dining area is stained; light intensity in walk-in freezer is too low.
Wendy’s, 5212 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures or a physical cleanup kit for cleaning vomit and diarrhea; to-go containers are not stored inverted to protect from contamination; observed grease dripping down the sides and undersides of deep fryers; shelf under burger grill is covered in grease and grime; walk-in cooler fan guards are dusty; there are unnecessary items stored in the dumpster area; ceiling tile in dining area is stained; observed grease and food debris/trash on floor under deep fryers.

Deep Run Roadhouse, 12379 Gayton Road – The following violations were reported during a routine inspection: temperature of top portion of brisket in steam well is too low; temperature of potato salad in prep unit is elevated; sanitizer concentration in spray bottle needs to be diluted; employee handling food is wearing bracelets; single cutting boards are heavily scratched and stained; there are holes in the wall; coving is separated from the wall behind front warmer/prep tables; areas of kitchen floor are no longer smooth and easily cleanable.
West Coast Provisions, 301-A Maltby Boulevard – The following violations were reported during a routine inspection: food protection manager certification is expired; raw burgers are stored above a tray of sweet potatoes in walk-in; observed black residue inside ice machine and black debris on ice; food residue found on dicer, slicer blade and blade guard, and multiple knives stored on magnetic strip at raw station; temperatures of pickled carrots and cucumbers in top of make table are elevated; pickled carrots in make table are past their expiration date; bread is stored uncovered on storage rack near cook line; unwrapped straws at beverage station are stored against a dusty ice machine vent; cutting board on cook line is heavily scratched and discolored; walk-in racks are rusty; pipe is leaking under prep sink; buildup of food debris found in floor drain near ice machine; dust is accumulating on walls and ceiling at beverage station; observed food debris and grease on floor at cook line.
Bubble Tea by Yogi Tea, 7801 West Broad Street – The following violations were reported during a routine inspection: there is 0 ppm sanitizer in bucket; observed dust buildup on hoods.
BB-Q Chicken, 7801 West Broad Street – The following violations were reported during a routine inspection: didn’t observe employee health policy or written procedures for cleaning up vomit and diarrhea; open employee drink found on top of food prep counter; raw chicken is stored over ready-to-eat rice; “clean” pans are stored with visible food debris on them; temperatures of rice and kimchi are elevated because they were left out on the counter; butane fuel is stored next to sauces on dry storage shelf; flour and spiced flour are unlabeled; observed buildup on pipes under three-compartment sink.
Joen Korean Restaurant, 7801 West Broad Street – The following violations were reported during a routine inspection: didn’t observe employee health policy or written procedures for cleaning up vomit and diarrhea; open Coke can found on food prep counter; employee washed hands in three-compartment sink without soap; employee handled cell phone and then handled clean utensils without removing gloves and washing hands; hand sink doesn’t have paper towels; raw pork stored next to sprouts and raw shell eggs stored over ready-to-eat foods in reach-in units; employee washed bowls, knives and a strainer without sanitizing (person in charge states they only use dish soap to wash dishes); fish cakes stored on front counter in small containers are not at the proper temperature; no date marks found on house-made kimchi or fish cakes; vitamins and medicines are stored next to food on dry storage shelf; observed many small flies near kitchen entrance; box of frozen squid is stored directly on floor below three-compartment sink; cell phone is stored on a cutting board; clean black rice bowls are stored under stove while still wet; plastic spoons are stored handle side down in open cup on counter; single-service boats are stored uninverted; there is a leak under the hand sink; wet mop isn’t hung or inverted to air dry; hood and floor behind stove are greasy.
Hanabi Japanese Restaurant, 7801 West Broad Street – The following violations were reported during a routine inspection: didn’t observe employee health policy or written procedures for cleaning up vomit and diarrhea; open employee drink found on prep shelf; person in charge touched cooked tempura shrimp with bare hands; hand sink doesn’t have hand soap or paper towels; facility couldn’t provide parasite destruction letter for sushi fish (yellowtail, salmon and tuna); raw chicken and shrimp are stored over ready-to-eat cabbage; microwave is dirty; “clean” knives have food debris on them; chicken on grill wasn’t cooked to the proper temperature; temperature of rice cooked yesterday in the reach-in cooler is elevated because of improper cooling methods (in a deep, covered container and cooled in the reach-in); temperature of bone broth on stove top is too low because the stove was turned off; rice found in microwave that person in charge states has been there since this morning; temperatures of tempura shrimp and rice are elevated because they were left out at room temperature; temperatures of chopped cabbage, sprouts and liquid egg in prep cooler are elevated (foods are overflowing from pans inside unit); no date marks found on chopped cabbage or cooked rice; didn’t observe a consumer advisory for sushi; large bucket of sugar is unlabeled; plastic single-service containers are being used as scoops in bulk sugar bucket.

Va House of Soul Cocktail Grill, 3406 Mechanicsville Turnpike – The following violations were reported during a routine inspection: mold found inside ice machine; dish machine isn’t dispensing soap or sanitizer; observed dried food debris and dust on reach-in cooler gaskets throughout kitchen and bar; grease residue found on sides of hot line equipment; there are stained and damaged ceiling tiles in dining area; there are cracked floor tiles in women’s restroom, dining area and kitchen.
Crafty Crab, 4995 Nine Mile Road – The following violations were reported during a routine inspection: there are no paper towels at sink near kitchen entrance, sink near deep fryers, bar hand sink or staff restroom; no handwash signage is posted at bar hand sink; bar hand sink doesn’t have hand soap; raw shrimp and oysters are stored over cornbread; raw lobster is stored over smoked sausage; raw shrimp is stored over steamed crab legs; pink mold found inside ice machine; employee cleaned pot and ladels but didn’t sanitize them; menu and to-go menu are missing a disclosure statement for raw oysters; observed fruit and drain flies in kitchen; food is stored directly on walk-in cooler, walk-in freezer and kitchen floors; smoked sausage, shrimp and mussels are stored uncovered in walk-in cooler; can opener base and blade and wire storage racks throughout kitchen are rusty; bar reach-in cooler door gaskets are torn; the interior is burnt and plastic is bubbling up inside microwave; reach-in coolers at bar area are in disrepair; cutting boards are heavily scored and stained; bulk sugar bin and lid are dirty and sticky; shelves above and underneath expo line are dirty, sticky and dusty; Pepsi reach-in cooler has sticky, dried beverage/food stains inside; sides of deep fryers and hood filters are greasy; bar area beer cooler door gaskets are dirty; shelf under bar area prep table is covered in sticky beverage residue; interiors of reach-in coolers used to store glasses are covered in sticky beverage residue; there is a leak in pipes underneath bar area hand sink; women’s restroom doesn’t have a covered trash can; there are dried food debris and stains on walls throughout kitchen; ceiling vent is dusty; multiple food items for staff are stored amongst food for customers and in food prep areas; light intensity in walk-in cooler and freezer is too low.

Tropical Smoothie Café, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: observed food debris and mildew buildup inside blender lids; employees with long ponytails and facial hair are not wearing the proper hair restraints; prep and single cutting boards are heavily scratched and stained; observed grease and food debris buildup on wire storage racks, monitor cables and drying mat for blenders.
Original Mexican Restaurant, 6406 Horsepen Road – The following violations were reported during a routine inspection: open drinks found on lowboy; observed mold buildup in soda nozzle; there is 0 ppm sanitizer at three-compartment sink; temperatures of white rice, yellow rice and cheese dip are too low; temperatures of lettuce, tomatoes, beef and pico in lowboy are elevated; temperature of salsa in ice bath is elevated; temperatures of beans, beef and cheese in walk-in are elevated; multiple spray bottles of chemicals are unlabeled; multiple raw and ready-to-eat items are stored uncovered in reach-in; raw steak in walk-in and tamales in freezer are stored uncovered; single-service items are not inverted or protected on prep line and in dry storage; walk-in racks are rusty; reach-in has a torn gasket; oven door is missing; there are gaps in hood filters; observed leak in pipe of hot holding unit.
Full Kee Restaurant, 6400 Horsepen Road – The following violations were reported during a routine inspection: person in charge could not verify employee food safety training; employee drinks found on prep tables; raw duck stored over green beans; eggs stored over ready-to-eat vegetables; duck stored over raw beef; observed dried food debris on duck hooks; temperatures of raw chicken, raw shrimp and bean sprouts are elevated; no date marks found on kimchi, pickled carrots or cooked noodles; tofu and dumplings are past their expiration dates; onions and utensils are stored next to dish machine chemicals; observed heavy ice buildup in reach-in freezer; walk-in cooler racks are rusted and some gaskets are torn; observed food debris and grime inside cold holding units and on storage shelves; food debris and grease found on wire racks in walk-in cooler; observed grease on rolling carts and under counter by cook line; there are grease and grime on floors and inside walk-in cooler; observed buildup on cooler fan; there is low grout on floors.

Zara Grill, 10865 West Broad Street – The following violations were reported during a routine inspection: employee health policy isn’t available; facility couldn’t provide written procedures for cleaning up vomit and diarrhea; an employee was prepping raw chicken and moved towards cook line to begin service without washing hands and changing gloves; first hand sink in the kitchen doesn’t have paper towels; raw meat kabobs are stored over ready-to-eat tomatoes, sliced cheese and pre-cooked gyro meat; container of insecticide found in kitchen (only licensed pest control professionals can apply pesticides); no food thermometer is available; lowboy freezer in back of kitchen has a broken door; gasket liner is torn on lowboy storing sauce; no sanitizer test strips are available; ingredient shelf above prep table is covered in a thin layer of grease buildup; no light shield is covering the string light bulbs over back prep table in kitchen.
