Honest Indian Vegetarian Restaurant, 8900-A West Broad Street – The following violations were reported during a routine inspection: employee entered kitchen from outside door and began food prep without washing hands; no handwash signage is posted at hand sink at front counter; temperature of pasta made yesterday in walk-in cooler is elevated because of improper cooling methods (stored in a deep covered container); temperature of rightmost samosa in warmer is too low; temperature of heavy cream in prep cooler on cook line is elevated; no date marks found on several curries, cooked vegetables and cheese in prep coolers; table cleaner in a spray bottle is unlabeled; spices at grill are unlabeled; sugar in bulk containers is uncovered on dry storage rack; single-service containers are being used as scoops in bulk sugar containers; singe-service to-go containers are not stored inverted or protected on prep cooler; gaskets on prep cooler in smaller kitchen are torn; floors and walls behind cook line equipment in large kitchen are dirty; walk-in cooler fan guards are dirty.
Project Lean Nation, 3442 Lauderdale Drive – The following violations were reported during a routine inspection: employee health policy is unavailable; back interior of the ice machine is surrounded by specks of black debris; temperature of almond milk in reach-in refrigerator is elevated; bulk bin containing powdered whey is unlabeled; boxes of prepared, prepackaged and vaccum sealed food items are stored directly on walk-in freezer floor.

Tava Cuisine of India, 3406 Pump Road – The following violations were reported during a routine inspection: soda nozzle in the bar has black buildup caked along the top interior and a small dark slime spot along the edge; dishwasher is dispensing 0 ppm chloride sanitizer; bowl of cooked rice is stored at room temperature on back counter in kitchen; spray bottle containing pink colored dish detergent is unlabeled; large pot of standing water on the floor next to the stove is storing two large serving spoons; wire racks in the dry ingredient's storage area are peeling on the bottom shelf; long cutting board on prep line is stained and scored; wire racks in walk-in cooler are covered in old food debris; walk-in cooler vents are covered in a thin layer of dust.
Pizza Hut, 3113 Mechanicsville Turnpike – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit and diarrhea; facility doesn’t have a bodily fluid cleanup kit on site; door gasket on proofer box is torn; observed dried food debris and lime scale on exterior of dish machine and lime scale on interior of dish machine; wire storage racks throughout kitchen are greasy, dusty and dirty; grease residue found on sides of deep fryer; pizza make table has grease residue and grime on exterior and interior; point-of-sale monitors and buttons have grease and dried food debris on them; walk-in cooler fan guards and ceiling are dusty; observed grease residue on walk-in cooler glass doors; back room floor is unsealed and no longer smooth and easily cleanable; observed dried food debris and grease on wall behind dish machine; there are food stains on walls near make table; ceiling tile are dusty, especially near ventilation; hood system exterior walls are dusty.

Sandston Bistro & Shakes, 18 East Williamsburg Road – The following violations were reported during a routine inspection: cardboard is being used to line shelves throughout the kitchen; some cutting boards have heavy scratches; dried food debris found inside cold units; observed grime and food residue under large kitchen equipment.
Bojangles, 5207 Brook Road – The following violations were reported during a routine inspection: observed mold and black stains inside ice machine as well as flour, mold and grime on outside of machine; temperatures of bo-rounds and pork BBQ are too low; container of degreaser is stored amongst food items; bulk containers of rice and seasoned flour are unlabeled; cutting board is scored and stained; observed grease residue on sides of hot line cooking equipment; wire dish racks above three-compartment sink are dusty; dried food debris found on blue push cart; observed flour and food debris on reach-in cooler door gaskets; there are dried food stains on walls behind biscuit make table, three-compartment sink and prep sinks; there are food and trash in floor drains; ceiling tile in dining room area is stained; raw chicken walk-in cooler ceiling and walls are dusty.
The Melting Pot, 9704 Gayton Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy; staff handled dirty dishes and then put on gloves to begin food prep without washing hands; black residue found in ice machine; there is 0 ppm chlorine in dish machine; clean fondue forks are stored above hand sink under soap dispenser; racks in reach-in freezer and walk-in cooler #2 are rusty; racks in walk-in cooler #2 are dirty; there is a y-connector at mop sink without the proper backflow prevention device installed.
New Asian Panda, 10430 Ridgefield Parkway – The following violations were reported during a routine inspection: back hand sink doesn’t have hand soap; temperatures of garlic in oil on cart and cut cabbage on counter are elevated because they are not stored in a cooler unit; prep unit cutting board is heavily scratched and stained; observed dried food debris and grease buildup on wire storage racks, prep shelves, spice cart, small appliances (instapot) and inside reach-in and prep units.
The Smoothie Spot, 1366 Gaskins Road – The following violations were reported during a routine inspection: did not observe employee health verification; hand sink next to three-compartment sink doesn’t have paper towels; there is 0 ppm quat sanitizer in three-compartment sink; person in charge is cloth drying the blender; there is a leak under the three-compartment sink; bathroom door was left open; baseboard is chipped at mop sink.
Mama Cucina, 4028-O Cox Road – The following violations were reported during a routine inspection: employee is handling sliced bread with bare hands; the only hand sink in the kitchen is blocked by a serving tray storing dirty dishes; an employee dropped a block of cheese on the floor and attempted to still use it; container of raw chicken is stored over ready-to-eat pasta in reach-in unit; the ice machine’s back interior shield is covered in a thick layer of discolored slime along the bottom; soda nozzle has black, caked-on buildup along the bottom of the interior; top shelf of prep unit storing take-out containers is covered in a layer of grease; gasket liners are torn on reach-in units #1 and #3, lowboy unit on prep line, both doors of lowboy unit in bar area, and right-side door of reach-in unit in bar area.

Wingstop, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: temperature of cheese sauce in dispenser is too low; employee with long facial hair isn’t wearing a beard guard; employees are wearing bracelets and rings while preparing food; observed buildup of dried food debris and grease on toaster oven, fryer touchpad and prep shelves; observed grime and food splatter on walls near three-compartment sink and inside floor drains; there is dust buildup on ceiling vents and panels; cement floor in back storage room/hallway is no longer smooth and easily cleanable.

Cracker Barrel, 3620 Mayland Court – The following violations were reported during a routine inspection: temperature of bagged hash browns in prep cooler is elevated; there is no time label for raw shell eggs out at the grill; spray bottle containing bleach is unlabeled; employee preparing food is wearing a watch; dish rack in dish room is rusted; staff bathroom doesn’t have a covered trash can; staff bathroom door was left open; there is dust buildup on ceiling above warming cabinet; observed grease buildup on floor next to fryer.
China Wok, 11530-C West Broad Street – The following violations were reported during a routine inspection: container of raw shrimp covered with cling wrap is stored over ready-to-eat cabbage in lowboy on prep line; temperature of shredded lettuce is elevated because it was left out of refrigeration; cooked chicken is stored in a non-food grade grocery bag in chest freezer; bucket of soy sauce is stored directly on walk-in freezer floor; gasket liners are torn on both doors of first prep unit on the line, middle and right-side door of second prep unit on the line, and on walk-in cooler door; long cutting boards on prep line are scored and stained; there is no approved sanitizing bleach on the premises; drying shelf above three-compartment sink is covered in a thin layer of dark debris; spray nozzle connected to mop sink faucet hose doesn’t have the appropriate backflow prevention device installed.
Urban Air Trampoline and Adventure Park, 10050 West Broad Street – No violations reported during a routine inspection.
Revel, 6223 Lakeside Avenue – The following violations were reported during a routine inspection: three plastic cutting boards have deep scratches and heavy stains; observed heavy ice buildup, broken plastic and torn gaskets on mini freezer on cook line; there is heavy dust buildup on multiple air vents on ceiling throughout kitchen.
Famous Fried Chicken & Seafood, 3050 Williamsburg Road – The following violations were reported during a routine inspection: person in charge talked on his cell phone and then proceded to prepare food without washing hands and changing gloves; observed leak from ceiling above hood.
